Specialist note IQTIG Eine Risikoadjustierung hat das Ziel, die unterschiedliche Patientenstruktur in verschiedenen Einrichtungen auszugleichen. Dies führt idealerweise zu einem fairen Vergleich der verschiedenen Einrichtungen, da Patientinnen und Patienten individuelle Risikofaktoren aufweisen (wie zum Beispiel Begleiterkrankungen), die das Qualitätsergebnis systematisch beeinflussen, ohne dass einer Einrichtung die Verantwortung für z.B. daraus folgende häufigere Komplikationen zugeschrieben werden kann. Mithilfe der Risikoadjustierung kann das Qualitätsergebnis beispielsweise einer Einrichtung mit vielen Hochrisikofällen fairer mit dem Qualitätsergebnis einer Einrichtung mit vielen Niedrigrisikofällen statistisch verglichen werden. Der Referenzbereich gibt an, bei welchen Ergebnissen von guter Versorgungsqualität einer Einrichtung ausgegangen werden kann, wobei Abweichungen möglich sind. Der Referenzbereich legt somit einen Maßstab für die Bewertung von Einrichtungen fest. Ein Ergebnis außerhalb des Referenzbereichs gilt zunächst als auffällig. Es ist zu beachten, dass ein Ergebnis außerhalb des Referenzbereichs nicht gleichbedeutend ist mit einer mangelnden Qualität der Einrichtung in dem betrachteten Qualitätsaspekt. Die Abweichung kann z. B. auch auf eine fehlerhafte Dokumentation, eine medizinische Besonderheit des Patientenkollektivs oder auf Einzelfälle zurückzuführen sein. Im sogenannten Stellungnahmeverfahren wird das auffällige Ergebnis analysiert und eine Bewertung der Qualität vorgenommen.
