Das Kreiskrankenhaus Prenzlau erarbeitet im Rahmen des Disease-Management-Programms Diabetes Typ 2 (DMP) für jeden Patienten eine entsprechende Therapie. Sie beinhaltet die medikamentöse Behandlung und ausführliche Schulungen und Anleitung der Patienten. Ziel des DMP ist die Optimierung der Kooperation aller am Behandlungs- und Betreuungsprozess von Patienten mit Diabetes beteiligten Institutionen für eine optimale Diagnostik und Therapie unter Einhaltung der festgelegten Leitlinien.
